In a recent episode of Khan Klinics, host Dr. Amir Khan welcomed Dr. David Burns, a pioneer in psychotherapy. A Stanford professor, Dr. Burns, has helped millions "untwist their thinking" to find emotional freedom. His work includes award-winning research and bestsellers such as "Feeling Good: The New Mood Therapy," which sold over 4 million copies. This talk offers a path to inner peace.
The Birth of a Revolution: Understanding CBT

Dr. Khan initiated the discussion by asking about the essence of Cognitive Behavioral Therapy. David Burns explained that "cognition is just a fancy word for a thought". CBT helps you understand how your thoughts and feelings are connected to your actions, allowing you to change unhelpful patterns. Its main idea is 2,000 years old, from the Greek philosopher Epictetus. who stated, "we're disturbed not by events, but by our thoughts about them".

Understanding TEAM CBT

TEAM CBT stands for Testing, Empathy, Assessment of resistance, and Methods.
Dr. Burns called it "cognitive therapy on steroids" and a "super powerful version."Regular therapy started in the 1970s and 80s and was the first drug-free way to treat depression, often in just 5 to 15 sessions. But it didn't work for everyone.

Dr. Burns said a main reason for not working for everyone was "resistance." This means patients might want to get better, but some of them unknowingly fight against help. Dr. Burns claimed that “TEAM-CBT uses methods to reduce and eliminate that resistance and boost motivation." This leads to "ultra high speed treatment and recovery."

The Pillars of TEAM: T.E.A.M. Explained

Dr. Burns elaborated on the four pillars of TEAM-CBT:

T - Testing: This involves testing patients at the start and end of every session with "brief, highly accurate tests". These tests measure depression, suicidal urges, anger, anxiety, relationship issues, and even happiness. This innovation makes therapists accountable and sends a clear message: change is expected now.

E - Empathy: Dr. Burns acknowledged Carl Rogers' famous emphasis on empathy. Dr. Burns' research on digital Empathy shows that empathy alone "doesn't cure anything". He stated, "All the empathy in the world won't help you" if you have a serious condition. Empathy serves as a crucial first step, "a way to open the door," but "you still have to walk through the door to get into the house," where healing occurs.

A - Assessment of Resistance: This is a pivotal, often paradoxical, step. Therapists help patients become aware of the part of themselves that resists getting better.

M - Methods: Once resistance is addressed, Dr. Burns utilizes over a hundred techniques "to smash these distorted negative thoughts that trigger depression, anxiety, anger, hopelessness, shame, and guilt.”

The Speed of Recovery

Traditional therapy often takes months to years to show results. A remarkable claim from Dr. Burns is the speed of recovery. Dr. Burns  stated that recovery "always happens in about 30 seconds." Even though it might take a "couple of hours to build up to the moment", the instant you stop believing your negative thoughts, "in that instant, you'll recover".

This quick change is the result of "40 years of therapy, research, and innovation" in the TEAM package. Dr. Burns was excited that "people who are suffering can hope for dramatic change and rapid dramatic change."

The Daily Mood Log – The “Feeling Great” App

Dr. Burns also discussed fascinating developments in digital empathy, particularly through his "Feeling Great" app. This app features a digital version of himself, which he revealed "worked better than human empathy" and is "beyond what 99% of humans can do." Users consistently report being "shocked and blown away by how empathic the computer was."

The app is specifically designed to guide users through a daily mood log process. This powerful technique helps individuals pinpoint exact moments of distress, recognize their recurring emotional patterns, and directly challenge the negative thoughts that contribute to their suffering. Remarkably, Dr. Burns noted that most users experience complete symptom elimination within just two hours of using the app. For more information, click here.

Universal Application of TEAM CBT

TEAM CBT works across all cultures and backgrounds. Regarding background, beliefs, and culture, Dr. Burns believes "there aren't many social and cultural factors that have any impact at all." He shared experiences from an inner-city hospital, where he worked with a diverse group, including homeless patients, many unable to read or write.

Dr. Burns observed that "these distorted thinking patterns seem to be inherent... in every culture," from Philadelphia to China, where young people have similar thoughts like "I'm not successful enough" or "I'm worthless."

Final Stitch: Reclaiming Emotional Freedom

Dr. Burns closed the insightful discussion with a profound, three-part message that he calls "three principles" for well-being. Dr. Burns emphasized that not all of our feelings are simply due to what's going on in our lives, but are fundamentally influenced by our thoughts about those events

Dr. Burns explained that the negative thoughts we tell ourselves when feeling anxious, panicky, worthless, inadequate, guilty, ashamed, alone, hopeless, or angry are deceptive. "Those are cons. They're not true.” He said. Dr. Burns revealed that "the very moment you challenge and crush those negative thoughts and stop believing them, in that very moment, your feelings will change."

Dr. Amir Khan added, "Yield before you react and never underestimate the power of pause."
